  • Photo ID or driver's license
  • Insurance card or worker's compensation information/authorization
  • Cash, check, or credit card to pay for your co-payment or deductible requirements
  • Arrange in advance for an adult to drive you home and remain with you for the first 24 hours after surgery. You will not be allowed to have your procedure without someone to drive you home.
  • Please notify your surgeon before coming to the Center if there is any change in your physical condition, such as a fever or cold.
  • Be prepared to sign a form giving your consent for the procedure. If you are younger than 18, a parent or legal guardian must accompany you to sign the consent form.
  • Arrive promptly at your designated time
  • Your family, friends, and companions will be asked to wait in the waiting room. The surgeon will speak with your family immediately following the procedure.
  • Before you leave the Center, you will be given verbal and written instructions for your care at home. After you have returned home, please follow your doctor’s orders regarding diet, rest, medication, and activity
  • If you have any questions or concerns after your procedure, please contact your doctor immediately.

Our hours of operation are from 8:00 a.m. to 4:30 p.m., although we can open earlier and stay open later to accommodate you.

Please avoid all food and beverages beginning at midnight prior to your surgery or follow doctor’s instructions.

  • On the day of your surgery, be sure to wear loose-fitting clothing, making it easy to dress and undress yourself.
  • Remove all make up. Do not use lotions or oils after bathing
  • Leave all jewelry, dentures and contact lenses at home.

Yes, you will. Due to anesthesia, you will not be permitted to drive for 24 hours after your surgery.

After changing, all of your belongings will be safely stored in the pre-operative area.
